Pleural effusion

 

Pleural effusion is a frequent finding associated with cardiomegaly as in this case.  Pulmonary vascularity is also increased.

The black arrows denote the curvilinear density representing significant fluid at the right lung base.  There is likely a small amount of fluid on the left as well.
